This notice applies to the public WIDE website and its founding-place and professional enquiry forms.

Who is responsible for the information?

WIDE Rehab Evidence Ltd is the data controller for public website enquiries. Contact: nicholas@movewide.co.uk.

What the public forms collect

The founding-place form collects contact details, a short description of one movement or activity and the reason it matters. The professional form collects contact, role and organisation details and a brief non-identifiable enquiry summary.

Please do not send full medical records, photographs, attachments or identifiable patient information through these public forms.

Why the information is used

WIDE uses the information to respond to the request, decide whether a possible next step exists and arrange a secure clinical or professional process where appropriate. The public founding-place form is not itself a clinical assessment or record.

How the forms are delivered

When submitted, the form content is sent to the WIDE email address. If the website mail function fails, the page will ask you to contact WIDE directly instead.

Retention

Public enquiries are kept only as long as needed to respond and manage any appropriate follow-up, normally no longer than 12 months unless a clinical or contractual relationship begins or a legal reason requires longer retention.

Sharing

WIDE does not sell public enquiry information. It may use service providers needed to host the website or deliver email, subject to appropriate data-protection arrangements. Information will otherwise be shared only where required by law or with your agreement.

Your rights

You may ask for access, correction or deletion of your information, or object to its use, by emailing WIDE. You may also complain to the UK Information Commissioner’s Office.

Cookies

The WIDE website does not currently use analytics, advertising or behavioural-tracking cookies. Essential hosting technologies, security measures and server logs may still be used to deliver and protect the site. Because no non-essential tracking is currently used, the site does not display a cookie-consent banner. This will change before any non-essential analytics or advertising technology is introduced.

Clinical privacy

If a First Look progresses, WIDE will use a separate secure clinical intake, consent and record-keeping process. The public request form should not be used for urgent care or confidential clinical documents.
